This map shows hospitals that are dedicated to caring for patients with substance use disorders. Each hospital is part of a treatment network supported by our community partners listed below. Through collaboration, we are able to offer patients a full range of addiction treatment and recovery services local to the Eastern Plains. This is not an exhaustive list of all treatment providers in the community, but these providers have collaborated with the CO ROOTS team and will provide quality care to patients seeking treatment and recovery.

Connect- CARE coaches bring empathy, support and lived experience to every client served, helping to establish safety and trust from which a relationship can grow.
Discover- CARE coaches work closely with clients to understand their challenges and goals. Through experience sharing and applied skills, coaches help clients to discover a clear path forward.
Bridge- coaches bridge clients with vetted organizations to ensure they connect with and receive the resources needed to sustain change.

Peer support
Continuity of care and transitional support for incarcerated persons

Peer Support- Peer recovery coaches walk side by side with individuals seeking recovery from substance use.
Family Support- Provide community, resource navigation, and a pathway for those affected by a loved one’s substance use. Through one on one coaching, CRAFT groups, support meetings, and a community, individuals can begin their own healing.
All Recovery Meetings-All are welcome, as AFRC celebrates and supports all pathways of recovery in our community of solutions.
