Colorado ROOTS is made possible by the generous sponsorship of Northeast Health Partners, whose investment in rural addiction care has made this addiction care system possible. Our hospital partners and their dedicated medical staff will help revolutionize care on the Eastern Plain in close partnership with our addiction care and peer support partners. Together, we will create an effective, equitable and sustainable system of addiction care for our communities.
