This Privacy Policy describes Our policies and procedures on the collection, use and disclosure of your information when you use our care coordination and warmline services and tells you about your privacy rights and how the law protects you. No contact phone number or SMS consent will be shared with third parties.

Definitions

For the purposes of this Privacy Policy:

While using our care coordination and/or warmline services, We may ask you to provide us with certain personally identifiable information that can be used to contact or identify you or your patients. Personally identifiable information may include, but is not limited to:

  • First name and last name

  • Phone number

Squarespace websites use essential cookies and non-essential cookies. The essential cookies are necessary for the functioning of the website. Privacy laws do not require you to obtain consent to use them. You can use essential cookies without anyone's permission.

When calling or texting the Colorado ROOTS warmline, your phone number will be recorded automatically but will not be shared with external parties, except when consent has been given to share with the Colorado ROOTS referral network. Your personal information will never be sold to third parties or used in marketing or promotions. 

Use of Your Personal Data

Colorado ROOTS may use Personal Data for the following purposes:

  • To provide care coordination and referrals services, including to monitor the usage of our services.

  • For the performance of a contract with Colorado ROOTS in providing care coordination, referral and warmline services, in compliance with HIPAA and 42 CFR Part 2, by which we are bound.

  • To contact you: To contact you by email, telephone calls, SMS, or other equivalent forms of electronic communication regarding updates or informative communications related to the functionalities or contracted services when necessary or reasonable for their implementation. You will only be contacted via SMS if you initiate the SMS conversation, or give verbal consent to receive a text message.

We may share you or your patients personal information in the following situations:

  • With treatment and recovery service providers: We may share you or your patients personal information with treatment and recovery service providers for the purposes of care coordination and referral, with the consent of the patient.

  • With Business Associates: We may share Your information with our business associates (Colorado ROOTS partners), in which case we will require those affiliates to honor this Privacy Policy.

  • With your consent: We may disclose your personal information for any other purpose with your consent

Retention of Your Personal Data

Colorado ROOTS will retain your Personal Data only for as long as is necessary for the purposes set out in HIPAA and 42 CFR Part 2. We will retain and use your Personal Data to the extent necessary to comply with our legal obligations.

Law enforcement

Under certain rare circumstances, Colorado ROOTS  may be required to disclose your Personal Data if required to do so by law or in response to valid requests by public authorities (e.g. a court or a government agency).

Colorado ROOTS may disclose your Personal Information  in the good faith belief that such action is necessary to:

  • Comply with a legal obligation

  • Prevent or investigate possible wrongdoing in connection with our services

  • Protect the personal safety of Users of the Service or the public

Referral partners and business associates have access to your Personal Data only to perform services under Colorado ROOTS, and are obligated not to disclose or use it for any other purpose.
